Nature of the service

This service is a messaging-based teleconsultation for non-emergency concerns. A doctor will review the information you provide and may decide that messaging is not suitable or recommend voice, video, in-person, urgent or emergency care instead.

Emergency and urgent concerns

This service must not be used for an emergency or urgent medical concern, severe or rapidly worsening symptoms, or a concern that may need immediate physical examination. If any of those apply, stop this flow and seek urgent care.

Clinical outcomes are not guaranteed

The service covers a doctor’s review, assessment and advice. A prescription, referral, laboratory request, medical certificate, fit-to-work certificate, diagnosis, document or any other specific outcome is not guaranteed and will be provided only when clinically appropriate.

Use of your information

You consent to My Doc Express collecting and processing the personal and health information you provide for this consultation. This includes the eligibility answers and later consultation information you choose to submit.

Accuracy

You confirm that the information you provide will be true and accurate. Incomplete or inaccurate information may limit what the doctor can safely assess or recommend.

Consent record

Your acceptance is recorded before eligibility answers, clinical intake or uploads are stored. The record includes the wording and notice version you accepted, the acceptance time, the applicable privacy and terms versions, and available technical request details such as IP address and browser user agent.
