I confirm that I am 18 years old or older and requesting this consultation for myself. I understand that this is a general teleconsultation for non-emergency concerns, using the consultation option I select in the service flow. A doctor may decide that the selected option is not suitable or recommend another type of care, and prescriptions, referrals, laboratory requests, medical certificates, fit-to-work certificates and other outcomes are not guaranteed. I consent to My Doc Express collecting and processing my personal and health information for this consultation, and I confirm that the information I provide will be true and accurate.
Nature of the service
This service is a general teleconsultation for non-emergency concerns. The consultation option shown in the service flow may use secure messaging or, when available and selected, a voice call through your web browser. A doctor will review the information you provide and may decide that the selected option is not suitable or recommend voice, video, in-person, urgent or emergency care instead.
Emergency and urgent concerns
This service must not be used for an emergency or urgent medical concern, severe or rapidly worsening symptoms, or a concern that may need immediate physical examination. If any of those apply, stop this flow and seek urgent care.
Clinical outcomes are not guaranteed
The service covers a doctor’s review, assessment and advice. A prescription, referral, laboratory request, medical certificate, fit-to-work certificate, diagnosis, document or any other specific outcome is not guaranteed and will be provided only when clinically appropriate.
Use of your information
You consent to My Doc Express collecting and processing the personal and health information you provide for this consultation. This includes the eligibility answers and later consultation information you choose to submit.

Accuracy
You confirm that the information you provide will be true and accurate. Incomplete or inaccurate information may limit what the doctor can safely assess or recommend.
Consent record
Your acceptance is recorded before eligibility answers, clinical intake or uploads are stored. The record includes the wording and notice version you accepted, the acceptance time, the applicable privacy and terms versions, and available technical request details such as IP address and browser user agent.
Voice call consultation addendum
This additional notice applies only when you choose the voice call consultation option.
I agree to a voice call with a doctor through my web browser. I need an internet connection and a device with a microphone. The call may not connect. Calls are not recorded.
Voice call consultation
The voice call consultation is a voice call through your web browser. We email you when the doctor is ready. Open your consultation link, select Join voice call and permit the microphone. Keep the page open until the call ends.
You need an internet connection, a phone or computer with a microphone, and a recent version of Chrome, Safari, Edge or Firefox. Use a quiet, private place where you can speak safely.
The call may not connect. Calls are not recorded by My Doc Express. A doctor may recommend another type of care. A prescription, referral, laboratory request, medical certificate, diagnosis or other specific outcome is not guaranteed.
Do not wait for the call if your condition is urgent or getting worse. Seek urgent or emergency care instead.
Identity and location
The doctor will ask you to confirm your identity and current location before discussing your health information. Do not allow another person to use your voice call consultation.
